One-on-one supplemental mental health services designed to change targeted behaviors through skills development with the youth and caregiver, as well as an introduction to outside resources that support sustained change. The TBS program works in collaboration with the youth’s mental health team at the home, school, and other community locations throughout Sacramento County.
Description of Services
- TBS works together with families to iden tify and work through specific challenges (communication, behavior, etc.)
- TBS works with families to help create a plan to help with challenges and increas ing family health
- TBS helps families learn ways to maintain family wellness
Eligibility Requirements
- Under 21 years old, and
- Currently receives Medi-Cal outpatient mental health services.
Plus at least one of the following:
- Living or at risk of living in a group care setting
- Stepping down from a Short Term Residential Therapeutic Program
- Received treatment at the Sacramento County Mental Health Treatment Center in the past 6 months
- Had a psychiatric hospital stay in the past 24 months
- At risk of a psychiatric hospitalization
