Transitional Age Program
Sacramento Children’s Home is pleased to be able to offer three different
community based programs. One of these programs is TAP, which stands for
Transitional Age Program, and is capable of annually serving the needs
of approximately 75 youth/young adults and their families. This program
is directed towards clients aged 12 to 21 who have been in mental health
system and are at risk for homelessness, psychiatric hospitalization,
and high levels of care. The program works with clients to give them independent
living skills such as finding appropriate housing, developing vocational
goals, budgeting and house hold management, recreation planning, increasing
their coping skills, medication management and education in order to keep
them in the least restrictive normalized living environment possible.
This is accomplished by means of providing therapy, case management, and
rehabilitative mental health services.
CPS/School Intervention Program
The second program is CPS/SIP and its goal is to improve the client’s
overall social and emotional functioning. Both programs serve clients
that have behavioral symptomatology that interferes with their ability
to be successful in school and in their current home environment. CPS/SIP
helps their clients achieve their goals by mean of services that include
assessment, individual therapy, social rehabilitation, group therapy,
case management, medication support, and crisis intervention services.
Services are strength-based and incorporate and inclusive process of treatment
involving families. The CPS portion of the program provides mental health
services to victims of child abuse and severe neglect, while the SIP (School
Intervention Program) provides a combination of in-office and in-home/in-school
mental health services.
Family Resources Centers
The third community based program offered through Sacramento Children’s
Home is the Family Resources Centers and Birth and Beyond Program. The
Sacramento Children’s’ Home operates Family Resources Centers and Birth
and Beyond Program at three locations: North Sacramento, Valley Hi, and
Meadowview. Expectant or new mothers are referred for a variety of services
and services can continue until their child is 5 years old. The program’s
goal is early prevention - to prevent child abuse before it begins and
to keep the families out of the CPS system by enabling parents to provide
a safe environment for their children. In addition to the in-home services,
there are a variety of center based services available at resource centers
to support families in the following areas: child development, child abuse
treatment, mental health, alcohol and drug treatment, public health, and
CalWORKS. The Family Resources Centers also provide families with a variety
of programs including health and wellness classes, employment training,
recreational and social activities, parent support groups, and child development
groups.
